My request is stuck on AwaitingResponse.

The workflow is waiting on an external party, typically a vendor, to complete a tokenised form. Open the request to see who, and resend the link from the request page if needed.

The form will not let me submit because of a "duplicate" warning.

Duplicate detection is informational, not blocking. Read the matched record. If it really is the same, choose Raise update instead. If it is unrelated, dismiss the banner and submit anyway.

I uploaded a file but the approver says they cannot see it.

Open the request and check the Documents section is populated. If the file is still uploading you will see a progress indicator. Re-upload if necessary.

Why does the request show "ActiveInERP" but I cannot find the record in BC?

Ask an administrator. The audit trail records the response BC returned when the record was created, including the BC entity id, which they can use to find it directly in BC.
